Action Van

Action Van is an established service that helps to facilitate the timely discharge of patients from the hospital setting back to living in their own homes.  This is achieved through the provision and installation of rails / banisters and essential items of equipment without which a potential delay in a hospital discharge could occur.

During 2009, this valuable service facilitated over 200 hospital discharges from St. Vincent’s University Hospital, St. Michael’s Hospital, St. Columcille’s Hospital and The Royal Hospital Donnybrook.  In conjunction with this, the Action Van service worked collaboratively with Community Occupational Therapists facilitating the collection and delivery of equipment to over 350 people living in their own homes.

The Action Van Service aims to:

• Expedite hospital discharges
• Improve the quality of hospital discharges
• Improve the safety of a patient’s home environment reducing the risk of falls, etc.
• Increase an individual’s level of independence
• Ease the strain on family members and carers
